Responsibilities Include:
- Providing home-based care to patients under the supervision of an RN
- Working with a multidisciplinary team to deliver quality patient care
- Documenting patient progress and reporting changes to the RN supervisor
- Assisting the RN in carrying out the patient's plan of care and teaching self-care techniques
- Educating families on patient care, medication, and home safety
- Collaborating with the interdisciplinary team to coordinate patient care and plan for discharge
Salary : $55,000 - $65,000
