We are seeking a Registered Nurse to join our Hospice division in West Michigan. This role is ideal for professionals who value flexibility and work-life balance.
The successful candidate will be responsible for completing ongoing assessments of patients' physical status and psycho-social needs. They will develop and implement plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and their families.
RN/LPN Responsibilities:
- Initiate and develop a plan of care in collaboration with the IDG, patient/family, and attending physician
- Provides education and interventions to support effective management of patients' pain and other physical symptoms
- Ensures that equipment, supplies, drugs, and biologicals are available as needed
- Make necessary revisions to the IDG plan of care as patients'/families' status and needs change
- Completes accurate and timely documentation of patient/family services provided
- Regularly attends IDG meetings
RN/LPN Requirements:
- RN/LPN with active, unrestricted license in the State of Michigan
- At least one year of relevant experience as an RN or LPN
- One year of management experience preferred
- One year of home health experience preferred
- Current CPR Certification
- Community health and wide range of Medical-Surgical experience preferred
- Reliable transportation with valid state driver's license and current car insurance
