Job Description
As a Registered Nurse or Licensed Practical Nurse with Interim HealthCare, you will play a vital role in our interdisciplinary team, working closely with patients, families, and healthcare providers to deliver seamless and compassionate care. Your expertise will be essential in assessing patient needs, developing individualized care plans, and ensuring timely and effective interventions.
Responsibilities
- Conduct thorough assessments of patient physical status and psycho-social status to inform care planning
- Collaborate with the Interdisciplinary Group (IDG) to develop and implement personalized care plans
- Provide education and support to patients and families on symptom management and self-care
- Ensure timely ordering and availability of necessary equipment, supplies, and medications
- Maintain accurate and up-to-date documentation of patient services provided
Requirements
- Active, unrestricted RN or LPN license in the State of Michigan
- Minimum one year of relevant experience as an RN or LPN
- Prior experience in home health or hospice care preferred
- Current CPR certification
- Reliable transportation and valid driver's license
