Job Description
Our Home Health Registered Nurses play a vital role in providing personalized care to patients with illnesses, injuries, and chronic diseases. Your responsibilities will include:
- Providing home-based care to patients after discharge from hospitals, rehabilitation centers, or nursing facilities
- Working with physicians and overseeing LPNs, CNAs, and aides to ensure seamless patient care
- Examining patients, conducting assessments, documenting progress, and reporting changes to physicians
- Educating patients on their plan of care, prescribed medication, therapy, diet, and exercise
- Cooking family members on the patient's plan of care, medication, and home safety
- Ensuring goals are met and coordinating discharge from services
Required Skills and Qualifications
To succeed in this role, you will need:
- A valid, unrestricted RN license in Michigan
- At least one year of clinical experience as a Registered Nurse
- Current CPR certification
- Reliable transportation with a valid state driver's license and current car insurance
