**Key Responsibilities:**
- Complete ongoing assessments of patients' physical and psycho-social status
- Develop and implement individualized care plans in collaboration with IDGs, patients, and attending physicians
- Provide education and interventions to manage patients' pain and symptoms
- Ensure seamless communication among healthcare team members
What You'll Need:
- RN/LPN with active, unrestricted license in Michigan
- At least one year of relevant experience as an RN or LPN
- Preferred: one year of management experience, home health experience, and community health experience
