As a Hospice Patient Advocate, you will play a vital role in supporting patients and their families through difficult times. Your responsibilities will include:
- Providing home-based social services to patients dealing with end-of-life issues
- Developing and implementing patient care plans in collaboration with the hospice team
- Monitoring patients' psychosocial conditions and identifying areas for improvement
- Conducting patient assessments, documenting progress, and ensuring patients meet their goals
- Providing counseling, community resource planning, crisis intervention, and advocacy
- Consulting with family and caregivers on patient plans of care and how to support their loved ones
We require our Hospice Patient Advocates to have a Bachelor's Degree in Social Work from a Council on Social Work Education-accredited program and be licensed to practice as a Social Worker in the state where employed.
