RN/LPN Responsibilities:
- Deliver personalized care to patients and families, addressing their unique physical, emotional, and social needs
- Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as needed
- Maintain accurate records of patient interactions and services provided
- Foster open communication among healthcare providers to ensure seamless care coordination
RN/LPN Requirements:
- Active unrestricted license in the State of Michigan (RNs) or current LPN licensure (LPNs)
- Minimum 1 year of relevant experience as an RN or LPN (required)
- Preferred qualifications: 1 year of management experience and community health experience
- CPR certification and reliable transportation required
