About the Role

Key Responsibilities
- Conduct thorough assessments of patients to identify their healthcare needs and develop personalized care plans.
- Work closely with patients, families, and healthcare teams to ensure seamless coordination of care.
- Provide education and support to patients and their caregivers on medication management, disease self-management, and other health-related topics.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive care and optimal outcomes.
- Maintain accurate and detailed records of patient interactions, progress notes, and care plans.
Requirements
- A valid Registered Nurse (RN) license in the State of Michigan.
- At least one year of experience as a registered nurse, preferably in home health or hospice care.
- Current CPR certification and reliable transportation.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
