Nurse Liaison Specialist

Responsibilities

  • Act as a contact person for hospice referral sources, outside agencies, healthcare professionals, and field staff.
  • Provide basic information to patients and families regarding access to services.
  • Communicate with patients and families about arrangements for initiating care.
  • Complete intake screenings, documenting and analyzing required information.
  • Log referrals and prepare reports.

Requirements

  • Graduate of an accredited school of nursing.
  • Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Michigan.
  • One year of experience as a LPN or RN.
