About the Role
The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care. As a Case Manager, you will work closely with patients, families, and healthcare providers to develop personalized care plans, ensure seamless transitions throughout the care continuum, and promote a culture of patient advocacy and empowerment.
Your expertise will include ongoing assessments, plan development, education on pain management and symptom assessment, and documentation of patient information. You will also participate in regular IDG meetings and contribute to ongoing quality improvement initiatives.
Key Qualifications
- RN/LPN with active, unrestricted license in the State of Michigan
- At least one year of relevant experience as an RN or LPN (required)
- One year of management experience (preferred)
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
