About the role:
As a Home Health RN Case Manager, you will be responsible for:
- Conducting thorough assessments and developing personalized care plans
- Collaborating with healthcare teams to ensure seamless care transitions
- Educating patients and families on disease management and self-care techniques
- Monitoring patient progress and adjusting care plans as needed
- Communicating effectively with patients, families, and healthcare providers to ensure optimal outcomes
Requirements:
- Active, unrestricted RN license in the State of Michigan
- At least one year of clinical experience as a Registered Nurse
- Current CPR certification and reliable transportation
- Ability to work independently and as part of a team
