Physical Therapist Assistant Essential Functions
- Participates in ongoing interdisciplinary assessment and care planning
- Works under the direction of a treating physical therapist and in accordance with the established plan of care
- Assists in developing and evaluating the plan of care in partnership with patients, representatives, and caregivers
- Provides ordered services as indicated in the plan of care
- Supports the treating physical therapist in documenting patient progress and response to treatment
- Completes accurate and timely documentation addressing treatment, interventions, patient response, and progress towards goals
- Counsels and educates patients and caregivers to promote progress and meet healthcare-related needs
- Effectively communicates with healthcare team members to coordinate care and plan for discharge
- Communicates with physicians and other healthcare practitioners related to the current plan of care
- Accesses necessary personal health information to fulfill duties and responsibilities.
Job Requirements
- Current licensure as a physical therapist assistant in the state of Michigan
- At least two years of experience as a physical therapist assistant in the last five years
- Active CPR certification
- Previous home health experience preferred
