Job Responsibilities:
- Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
- Work collaboratively as part of a home health team (RN, LPN, CNA, HHA, OT, SLP) focused on patient care and goals
- Observe movements, conduct assessments, document progress, and modify care plans as needed
- Assist patients with exercises and hands-on therapy to manage pain and increase mobility
- Educate patients on their plan of care, established goals, exercises, and self-care
- Coach family members on the plan of care, therapy goals, and safely assisting with exercises
Requirements:
- Graduate of an accredited Physical Therapist Program and active PT license in Michigan
- Minimum of 1 year of physical therapy experience, preferably in home healthcare within the last 5 years
- CPR certification
- Knowledge of state and federal home health regulations
- Strong clinical judgment, interpersonal skills, dexterity, and compassion
