As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes. Your responsibilities will include:

• Conducting home visits to assess patients' needs and develop personalized care plans
• Collaborating with physicians and other healthcare professionals to coordinate care
• Providing education and support to patients and their families on disease management and self-care
• Coordinating discharge planning to ensure seamless transitions between care settings
