Key Responsibilities
Your key responsibilities will include:
- Examining patients, doing assessments, documenting progress and reporting changes to their physician
- Educating patients on their plan of care, prescribed medication, therapy, diet and exercise
- Couching family members on the patient's plan of care, medication and home safety
- Ensuring goals are met and coordinating discharge from services
