**Key Responsibilities:**
- Develop and implement individualized plans of care for patients
- Conduct thorough assessments of patients' physical, emotional, and social needs
- Collaborate with healthcare teams to ensure seamless care transitions and effective treatment planning
- Provide education and support to patients and families on disease management, self-care, and healthy living practices
- Monitor patient progress, adjust treatment plans as needed, and communicate effectively with patients and healthcare teams
**What We're Looking For:**
- Registered Nurse (RN) with active, unrestricted license in the State of Michigan – BSN not required
- At least one (1) year of clinical experience as a Registered Nurse
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
Salary : $75,000 - $100,000
