Responsibilities of a Home Health Nurse include:
- Providing home-based care to patients with illnesses, injuries, and chronic diseases.
- Working with a team of physicians and overseeing LPNs, CNAs, and aides providing patient care.
- Examining patients, doing assessments, documenting progress, and reporting changes to their physician.
- Educating patients on their plan of care, prescribed medication, therapy, diet, and exercise.
- Coaching family members on the patient's plan of care, medication, and home safety.
- Ensuring goals are met and coordinating discharge from services.

Requirements for Home Health Registered Nurses include:
- An active, unrestricted license in the State of Michigan.
- At least one year of clinical experience as a Registered Nurse.
- Current CPR Certification.
- Reliable transportation with a valid state driver's license and current car insurance.
