Responsibilities of a Home Health Nurse include:

• Providing home-based care to patients with illnesses, injuries, and chronic diseases.
• Working with a team of physicians and overseeing LPNs, CNAs, and aides providing patient care.
• Examining patients, doing assessments, documenting progress, and reporting changes to their physician.
• Educating patients on their plan of care, prescribed medication, therapy, diet, and exercise.
• Coaching family members on the patient's plan of care, medication, and home safety.
• Ensuring goals are met and coordinating discharge from services.

Requirements for Home Health Registered Nurses include:

• An active, unrestricted license in the State of Michigan.
• At least one year of clinical experience as a Registered Nurse.
• Current CPR Certification.
• Reliable transportation with a valid state driver's license and current car insurance.
