Job Responsibilities:

• Provide home-based care to patients with various illnesses, injuries, and chronic diseases.
• Work collaboratively with physicians and oversee LPNs, CNAs, and aides providing patient care.
• Conduct thorough assessments, document progress, and report changes to patients' physicians.
• Educate patients on their care plans, prescribed medication, therapy, diet, and exercise.
• Support family members in understanding patients' care plans and home safety.
• Cultivate effective communication with patients and families to ensure goals are met and discharge planning is coordinated.

Requirements:

• Active, unrestricted Registered Nurse (RN) license in Michigan.
• At least one year of clinical experience as a Registered Nurse.
• Current CPR Certification.
• Reliable transportation with a valid state driver's license and current car insurance.
