Job Summary
In this role, you will lead a team of RNs and LPNs in providing high-quality patient care and coordination of services.
You will be responsible for mentoring and guiding your team members, ensuring they have the necessary resources and support to deliver exceptional patient care.
- Key Responsibilities:
- Supervise and mentor a team of nurses in delivering patient care and coordinating services
- Develop and implement policies and procedures to promote quality care and efficient operations
- Collaborate with the IDG to ensure seamless transitions and optimal patient outcomes
- Monitor and evaluate team performance, providing constructive feedback and coaching as needed
- Participate in regular IDG meetings to review patient progress and make adjustments to care plans
Requirements and Qualifications
To succeed in this role, you must have:
- An active, unrestricted RN license in Michigan
- At least two years of experience in home health, hospice, or a related field
- Proven leadership and supervisory skills
- Strong analytical and problem-solving skills
- Excellent communication and interpersonal skills
