Responsibilities
- Participate in ongoing interdisciplinary assessment of patients, collaborating with physical therapists, nurses, and other healthcare professionals to develop comprehensive plans of care.
- Act at the direction of treating physical therapists, ensuring seamless implementation of treatment plans and continuous evaluation of patient progress.
- Contribute to the development and evaluation of plans of care, working closely with patients, representatives (if applicable), and caregivers to ensure mutual understanding and alignment of goals.
- Deliver ordered services by physicians as indicated in plans of care, maintaining accurate documentation of treatment and interventions provided, patient response, and progress toward established goals.
Requirements
- Current licensure as a physical therapist assistant in the state of Michigan is essential.
- A minimum of two years of experience as a physical therapist assistant within the last five years is required.
- Active CPR certification is mandatory for this role.
- Previous home health experience is a significant plus, but not necessary.
