As a Home Health Speech-Language Pathologist, your key responsibilities include:
- Providing home-based speech therapy to patients with communication, swallowing, or cognitive difficulties due to an illness or injury
- Working as part of a home health team that may include an RN, LPN, CNA, HHA, PT, and OT, focused on the patient's plan of care and established goals
- Observing and evaluating a patient's ability to communicate, swallow, and process information
- Conducting patient assessments, documenting progress, and ensuring patient goals are met
- Assisting with mouth, breathing, and swallowing exercises, fluency, articulation, and reading
- Educating patients and families on the plan of care, therapy goals, safety practices, and self-care
Speech Language Pathologist (SLP) Essential Requirements:
- Graduate of an accredited Speech Language Program and active SLP license in Michigan
- At least one year of experience as an SLP in the last five years
- Good clinical judgment, strong interpersonal skills, resourcefulness, and compassion
- Previous Home Health experience is preferred
