What You'll Do
You will provide personalized therapy to patients in need of creative strategies to make daily activities easier. This may include exercises to improve fine motor skills and coordination, suggesting adaptive equipment, assessing fall risks and introducing strategies to improve home safety, and educating patients and their families on the plan of care and self-care.
You will work as part of a home health team, collaborating with other healthcare professionals to develop and implement individualized plans of care.
