Interim HealthCare of Oklahoma City is seeking a detail-oriented and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health Clinical Coordinator. In this dynamic role, you'll help manage and support the delivery of exceptional home health care services while remaining available for timely, same-day patient visits when needed.
You’ll be instrumental in ensuring a smooth care journey for our patients—supporting clinical excellence, promoting continuity of care, and stepping in with confidence when same-day visits are necessary to support patient needs.
What You’ll Do:
- Perform in-home patient visits including initial assessments, follow-ups, and urgent same-day visits as needed
- Collaborate with RN Case Managers, physicians, and the interdisciplinary team to coordinate care and ensure patient needs are met
- Provide clinical oversight and coordination for assigned patients, including scheduling and documentation
- Participate in daily team meetings and contribute to care planning and prioritization
- Maintain accurate, timely, and thorough documentation in compliance with agency policies and home health regulations
- Serve as a supportive clinical resource for field staff
- Licensed Practical Nurse (LPN) in the state of Oklahoma, in good standing
- Minimum of 1 year of experience in a clinical setting—home health experience strongly preferred
- Excellent assessment, critical thinking, and time management skills
- Strong communication and interpersonal abilities
- Flexibility to complete same-day home visits when required
