Responsibilities:
- Conduct comprehensive assessments of patients to identify their unique needs.
- Develop personalized care plans in collaboration with patients, families, and healthcare professionals.
- Provide skilled nursing services, including wound care and medication management.
Requirements:
- Active, unrestricted RN license in South Carolina or compact multi-state license.
- Minimum 12 months of accumulated experience as a registered nurse within the last five years.
- Preferred experience in hospice care.
