RN Case Manager

RN Case Manager 

Lewiston/Auburn Maine 

Monday - Friday 8:30am-5:00pm

The RN Case Manager will be responsible for managing and coordinating patient care in collaboration with physicians and other healthcare professionals. This role will involve assessing patient needs, developing care plans, and providing direct and indirect nursing care.

Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Collaborate with physicians, healthcare professionals, and support staff to ensure optimal patient outcomes
• Monitor patient progress and adjust care plans as necessary
• Provide direct nursing care, including administering medications, treatments, and procedures
• Educate patients and their families on disease management and treatment options
• Coordinate and communicate with various healthcare providers and community resources
• Document patient interactions, interventions, and outcomes accurately and timely

Requirements:

• Active and unrestricted RN license in the state of Maine
• Previous experience in case management or a related field
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Proficiency in electronic medical records and other relevant software
• Valid driver's license and reliable transportation
