Acute RN OR Social Worker Case Manager CCM Certified
We are seeking a RN OR Social Worker Case Manager with a CCM Certification through CCMC. Must have a minimum of 2 years of experience within the last 5 years in an acute setting. This unique assignment will focus on the development, creation, and reviewing of assessments/educational content for Case Managers. The ideal candidate will be a subject matter expert and have a deep understanding of practices, workflows, and a keen attention to detail to design comprehensive and effective evaluations for current and future case managers. You will be working with our client to write, create, and review assessments and/or educational content. This assignment is a PRN (as needed) remote role.
MUST BE CCM Certified through CCMC

As a RN or Social Worker Case Manager (SME), here is the big picture of what you'll do:
- Research, Develop, and write new assessments and educational content for case managers, tailored to the needs of the client.
- Collaborate with and work with content developers to reflect current best practices and regulatory requirements.
- Research and complete writing assignments as directed.
- Review and analyze industry trends to ensure assessments align with evolving standards of case management.
- Maintain a thorough understanding of operations and regulations to ensure the accuracy and relevance of all materials.
A few must-haves for our RN or Social Worker Case Manager (SME):
- A minimum of 2 years of experience as a Case Manger within the last 5 years in an acute setting.
- MUST BE CCM Certified through CCMC.
- Strong knowledge of hospice procedures, medication management, and regulatory standards.
- Excellent written and verbal communication skills.
- Excellent research skills
- High level of professionalism
- Ability to work independently and collaboratively with a multidisciplinary team.
- Attention to detail and a strong focus on accuracy.
