The Hospice Bereavement Coordinator plays a critical role in the hospice care team, providing emotional support and guidance to patients' families and caregivers. This is a unique opportunity to make a meaningful impact in our community while utilizing your skills and experience.
In this position, you will develop and implement grief support programs, facilitate support groups, and ensure that families receive ongoing bereavement care after the loss of their loved one. You will also collaborate with the Interdisciplinary Group (IDG) to develop an appropriate bereavement plan of care.
