Your Role:
As a Hospice Licensed Practical Nurse, you will be responsible for delivering quality patient care and working closely with our interdisciplinary team. This includes taking patient vitals, performing assessments, documenting progress, and reporting changes to the RN supervisor. You will also assist the RN in carrying out the patient's plan of care and educate family members on the patient's plan of care.
Requirements:
To be successful in this role, you will need:
- A graduate degree from an accredited practical nursing program and an active LPN license in Louisiana
- Hospice LPN experience required
- CPR certification, valid Driver's License, auto insurance, and a reliable vehicle
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing, strong communication skills, and compassion
