In this hospice nursing role, you will have the opportunity to make a meaningful difference in the lives of patients and their families. As a skilled Hospice Licensed Practical Nurse (LPN) with Interim HealthCare of Louisiana, you will provide quality patient care in the comfort of their own homes or facilities, working under the supervision of a Registered Nurse (RN). Your responsibilities will include taking patient vitals, performing assessments, documenting progress, and reporting changes to the RN supervisor, as well as assisting the RN in carrying out the patient's plan of care, educating the family on the plan of care, and collaborating with the interdisciplinary team to coordinate patient care.
Requirements:
- Graduate of an accredited practical nursing program and active LPN license in Louisiana
- Hospice LPN experience required
- CPR certification, valid Driver's License, auto insurance, and reliable vehicle
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing, strong communication skills, and compassion
