Our Vision:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our hospice care team. As a member of our interdisciplinary team, you will provide skilled nursing care and support to patients and their families in the comfort of their own homes or facilities.
Key Responsibilities:
- Work collaboratively with physicians, RNs, CNAs, and aides to deliver high-quality patient care
- Take patient vitals, perform assessments, document progress, and report changes to the RN supervisor
- Assist the RN in carrying out the patient's plan of care
- Educate family members on the patient's plan of care
- Participate in the coordination of patient care with the interdisciplinary team
