General Purpose:
The Chaplain is responsible for providing spiritual services to patients and their families in Interim HealthCare's Hospice program. This includes conducting assessments of spiritual needs, ensuring timely provision of spiritual services, and participating in the development of interdisciplinary plans of care.

Essential Functions:
- Conduct assessments of hospice patients' and families' spiritual needs
- Ensure timely provision of spiritual services by either providing them directly or coordinating with community clergy or spiritual counselors
- Provide direct spiritual services according to patients' belief systems and practices
- Participate in interdisciplinary group meetings to develop plans of care addressing spiritual needs
- Conduct religious services for patients, families, and staff as requested
- Liaise and support community clergy and spiritual counselors
- Document services provided and ongoing communication with community clergy and spiritual counselors in a timely manner
- Maintain records of spiritual services utilization and related activities for quality assessment and performance improvement
- Actively participate in patient and family care conferences and IDG meetings
- Provide consultation, education, and support regarding spiritual issues and care to IDG members and facility staff
- Recruit community clergy and/or spiritual counselors to meet patient and family needs
- Act as the hospice spiritual services community liaison, developing contacts and offering education to clergy, counselors, and congregations
- Provide funeral or memorial services for patients as requested
- Plan periodic memorial services for IDG members, volunteers, facility staff, and community clergy/spiritual counselors
- Primarily responsible for conducting or arranging memorial services associated with the hospice bereavement program
- Assist in supervising spiritual care volunteers when assigned to patients and families
- Complete other assignments as requested and assigned
Minimum Education and Experience Requirements:
- Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree)
- Experience working with death and dying, terminal individuals, and their families or caregivers
- Hospice experience preferred
Knowledge, Skills, and Abilities Required:
- Knowledge of the hospice philosophy of care
- Good verbal and written communication and organizational skills
- Able to work as a member of the interdisciplinary group
- Able to effectively communicate with clinical and non-clinical employees, patients, and family/caregivers
- Able to perform and prioritize multiple functions or tasks
- Able to effectively deal with change
- Able to provide proof of valid driver's license, if applicable to hospice policy
- Able to provide proof of valid auto liability insurance if assignment includes driving own vehicle
- Meets applicable federal and state health requirements to provide services to patients
- Meets requirements of federal and state criminal and abuse background checks as applicable
Working Conditions and Physical Requirements:
- Work is normally performed in patients' homes and/or inpatient settings such as hospitals or nursing homes
- Occasional exposure to communicable diseases, bloodborne pathogens, and potentially infectious or hazardous materials requiring safety precautions
- Able to constantly travel locally from assignment to assignment
- Able to constantly stand, walk, bend, stoop, squat, kneel, and reach freely
- Sufficient endurance to perform required physical and mental tasks over long periods
- Able to grasp objects with thumb in opposition to fingers of palm
- Visual acuity, dexterity, and hearing to perform required skills
- Able to work flexible schedule and/or evening/weekend hours as needed
- Light physical activity requiring occasional lifting, carrying, pushing, or pulling up to 20 lbs to carry out daily job functions
