Responsibilities:
- Conduct assessments of hospice patients' spiritual needs
- Coordinate spiritual services with community clergy and spiritual counselors
- Provide direct spiritual care according to each patient's belief system and practice
- Participate in the development of the interdisciplinary group (IDG) plan of care
- Document services provided and ongoing communication with community clergy and spiritual counselors
- Maintain records of spiritual services utilization and related activities
