\Job Description
\We are seeking a skilled Physical Therapist to join our team. As a Home Health PT, you will provide personalized therapy services to patients in their homes, helping them regain strength, manage pain, and achieve their goals.
\Key Responsibilities
\- Provide home-based physical therapy to patients with mobility, strength, and functionality impairments due to illness or injury
- Work collaboratively with RNs, LPNs, CNAs, HHAs, OTs, and SLPs to achieve patient goals and outcomes
- Observe patient movements, conduct assessments, document progress, and modify care plans as needed
Requirements
\- Graduate of an accredited Physical Therapist Program and active PT license in Kansas
- CPR certification
- Knowledge of state and federal home health regulations
