Job Summary
We are seeking a compassionate and experienced Chaplain to join our team at Interim HealthCare of Manhattan. As a Senior Hospice Care Provider, you will be responsible for providing spiritual support to patients and their families in the Manhattan area.

Responsibilities
- Provide spiritual support to patients and their families through regular visits and counseling sessions.
- Assess the spiritual needs of patients and develop personalized care plans to meet those needs.
- Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients and their families.
- Conduct religious services and ceremonies as requested by patients and their families.
- Maintain accurate records of patient interactions and provide timely documentation of care provided.
- Participate in ongoing education and training to stay current on best practices in spiritual care and hospice service delivery.
Requirements
- Graduate degree from an accredited seminary or school of theology.
- Minimum of 2 years of experience working in hospice care or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to travel regularly to patient homes and facilities as needed.
- Valid driver's license and proof of valid auto liability insurance if driving own vehicle.
- Meet applicable federal and state health requirements to provide services to patients as appropriate.
- Meet the requirements of federal and state criminal and abuse background checks as applicable.
