The Role
We are currently seeking a skilled and empathetic Hospice Certified Home Health Aide to join our team. In this rewarding position, you will have the opportunity to build strong relationships with patients and their loved ones, providing essential daily support with activities such as bathing, dressing, and eating. Your expertise will be invaluable in helping us deliver exceptional patient outcomes.
Your Responsibilities
- Provide hands-on care to patients, including assistance with daily living activities, hygiene, and nutrition.
- Monitor patient vitals, intake/output, and blood glucose levels, documenting any changes or concerns.
- Collaborate with our nursing team to develop and implement comprehensive care plans tailored to each patient's needs.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless coordination of care.
Requirements
- A High School Diploma or equivalent, active Home Health Aide certification, and current CNA certification in California are required.
- CPR certification and a valid California driver's license with auto insurance are also necessary.
- Six months of healthcare experience, preferably in hospice care, is preferred, but not mandatory.
