Job Summary
- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs to develop individualized care plans.
- Coordinate care team meetings to ensure collaboration among healthcare professionals involved in patient care.
- Educate patients and families about disease progression, symptom management, and end-of-life care options.
Responsibilities
- Provide compassionate end-of-life care, including symptom management and comfort measures.
- Maintain accurate documentation of patient assessments, care plans, interventions, and outcomes.
- Participate in quality improvement initiatives aimed at enhancing the quality of care provided to hospice patients and their families.
Requirements
- Graduate of an accredited nursing program and active RN license in California.
- Minimum of 1 year of hospice nursing experience.
- CPR certification and reliable transportation.
