As a Hospice Chaplain, here's a big-picture view of what you will do:
- Conduct assessments of Hospice patients and their families' spiritual needs
- Ensure the appropriate spiritual services are provided in a timely manner by either providing such services to the patient/family or coordinating the provision of services with community clergy or spiritual counselors.
- Primarily responsible for coordinating the assessment of grief following the death of the patient, determining the bereaved’s needs and ensure timely and appropriate services in response.
- Regularly attends IDG meetings reviewing active patients, and participates in the development of the IDG plan of care for active patients as indicated.
- Maintains records of spiritual services utilization and related activities as directed for quality assessment and performance improvement (QAPI), hospice program development, and policies and procedures review and revision.
- Participates in the development of the interdisciplinary group plan of care to meet identified spiritual needs.
- Provides funeral or memorial services for patients as requested.
Minimum Education & Experience Requirements:
- Graduate of accredited seminary, school of theology and/or religious sturdies program (BD, MDiv or equivalent theological degree.)
- Experience working with death and dying, terminally ill individuals and their families or caregivers.
