Responsibilities:
- Conduct comprehensive assessments of patient needs, identifying opportunities for improvement and advocating for patient-centered care.
- Develop and implement individualized care plans, incorporating input from patients, families, and interdisciplinary teams.
- Deliver high-quality nursing care, including medication management, wound care, and education on disease management and prevention.
- Collaborate with other healthcare professionals to ensure seamless coordination of care and promote optimal patient outcomes.
- Document all interactions and interventions in our secure electronic health record system.
Requirements:
- Graduate of an accredited school for RN and licensed to practice as a Registered Nurse in the state of Minnesota.
- Minimum of twelve (12) months experience working in an acute care or home health environment.
- Evidence of current CPR certification.
- Able to travel within a geographic area serviced by the office.
- Reliable transportation, valid drivers license, and proof of auto insurance is required.
