Responsibilities and Expectations
- Conduct thorough assessments of patients' needs and develop individualized care plans
- Provide education and support to patients and their families
- Collaborate with healthcare teams to coordinate care and services
- Document patient progress and maintain accurate records
Requirements and Qualifications
To succeed in this role, you must have:
- A graduate degree from an accredited school of nursing
- Licensure as a Registered Nurse in the state of Minnesota
- A minimum of twelve months experience working in an acute care or home health environment
- Evidence of current CPR certification
- Ability to travel within a geographic area serviced by the office
