**Key Responsibilities**
- Provide hands-on care to patients in their homes, including wound management, medication administration, and vital sign monitoring.
- Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams.
- Counsel and educate patients on self-care techniques, health management, and disease prevention.
