">Key Tasks
">- Conduct regular assessments of patients' physical and psychosocial status.
- Develop and update individualized plans of care in collaboration with the IDG and other healthcare providers.
- Coordinate care services with the IDG and other healthcare providers to ensure continuity of care.
- Document all patient interactions and care activities accurately and timely.
Requirements
">- Active licensure as a Registered Nurse in the state of Minnesota.
- At least one year of experience in hospice care.
- Ability to travel to patient homes within the assigned territory.
