About the Job
This full-time position offers a unique opportunity to work as a Physical Therapist in a home care setting. You will have a general territory within our service area where you will provide one-on-one in-home care to patients.
Responsibilities
- Evaluate patient function and performance and develop plans of care.
- Provide interventions and therapeutic treatment in accordance with physician-certified plans of care.
- Assess changes in patient status and modify plans of care as needed.
Requirements
- Current licensure as a Physical Therapist in the state of Minnesota.
- Two years of experience as a Physical Therapist in the last five years, relevant to the patient population assigned.
- Able to travel within the geographic area serviced by the office, valid drivers license, and proof of auto insurance.
- CPR certification.
