Work Schedule and Territory
Full-time position with regular hours: 8:00am - 5:00pm, Monday through Friday. Work location is primarily in the field, visiting patients in their homes within the Southeast Metro area, which includes cities south of Highway 94 and east of Interstate 35W.
Job Responsibilities
- Collaborate with an interdisciplinary team to provide patient-centered care.
- Travel within the designated geographic area to visit patients in their homes.
- Develop and evaluate individualized plans of care with patients, representatives, and caregivers.
- Provide services ordered by physicians as outlined in the plan of care.
- Counsel and educate patients and families on meeting healthcare needs.
