Licensed Home Care Physical Therapist Assistant (PTA)
Enjoy independence and flexibility in the field to care for patients in their home environment.
Hours: Full-time, Monday - Friday, 8:00am - 5:00pm
Territory: This position will work in the field helping patients in their home setting. The office is in Roseville, MN, and the general territory for this position would be the northeast metro, north of Highway 94 and east of Highway 65.
Pay and Benefits:
- Hourly Pay Range: $26.00 - $32.00
- $2,500 Hiring Bonus

A few must-haves for this position:
- Currently licensed as a Physical Therapist Assistant in the State of Minnesota. License is active and in good standing.
- Two (2) years of experience as a PTA preferred.
- Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance.
- CPR Certification
Job Responsibilities:
- Participate in the development and evaluation of the plan of care.
- Provides services that are ordered by the physician as indicated in the plan of care.
- Assists the treating Physical Therapist in documenting a patient's progress and response to treatment.
- Completes and submits timely, legible, accurate, relevant, and sufficient documentation addressing the treatment and interventions provided, the patient's response to the treatment and interventions, and the patient's progress toward goals in accordance with professional standards of practice, policy, and procedures, and payer requirements.
- Counsels and educates the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare-related needs.
