About the Job
We are seeking a skilled Physical Therapist to join our team at Interim HealthCare of the Twin Cities. As a Physical Therapist with our company, you will have the opportunity to make a meaningful difference in the lives of our patients. You will work independently, providing one-on-one in-home care to patients in the south-west section of the Twin Cities metro area.
Key Responsibilities
- Assess patients' needs and develop treatment plans in collaboration with physicians and other healthcare professionals
- Provide interventions and therapeutic treatments in accordance with physician-certified plans of care
- Conduct ongoing assessments and evaluations to determine changes in patients' status
- Determine whether planned services should be modified, performing re-evaluations and obtaining revision orders from physicians as indicated
