Job Summary:
Interim HealthCare of the Twin Cities is seeking a highly motivated and experienced Registered Nurse to join our Home Care team as an Admissions Registered Nurse. The successful candidate will be responsible for completing admissions and/or visits for our patients, working independently in the field with flexibility in scheduling patient visits.
Responsibilities and Requirements:
- Responsibilities: Conduct thorough assessments of patients' needs, develop individualized care plans, collaborate with healthcare teams to ensure seamless care transitions
- Requirements: Graduate of an accredited school for RN and licensed to practice as a Registered Nurse in the state of Minnesota, minimum of twelve (12) months experience working in an acute care or home health environment, evidence of current CPR certification
