We are seeking a skilled Licensed Practical Nurse (LPN) to join our home health team in Lawrence & Topeka, Kansas. As an LPN with us, you will experience a rewarding career where you can make a difference in patients' lives.
Key Responsibilities:
- Provide high-quality home-based care to patients with illnesses, injuries, and chronic diseases under the supervision of an RN.
- Collaborate with a multidisciplinary team, including physicians, RNs, CNAs, and aides, to deliver exceptional patient care.
- Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor.
- Assist RNs in carrying out patients' plans of care and teaching self-care techniques.
- Educate families on patient care, medication, and home safety.
- Work with the interdisciplinary team to coordinate patient care and plan for discharge.
Requirements:
- Graduate of an accredited practical nursing program and active LPN license in Kansas.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing, strong communication skills, and compassion.
