A Licensed Practical Nurse (LPN) provides home-based care to patients with illnesses, injuries and chronic diseases under the supervision of a Registered Nurse (RN).

Duties:
- Provide home-based care to patients
- Work with a multidisciplinary team
- Take patient vitals, do assessments, and document progress
- Assist RN in carrying out patient's plan of care
- Educate family on patient's plan of care and medication
- Collaborate with team to coordinate patient care
Requirements:
- Graduate of an accredited practical nursing program
- Active LPN license in Kansas
- CPR certification
- Knowledge of state and federal home health regulations
