Your Role as a Home Health Physical Therapist
You'll be responsible for:
- Providing home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
- Working collaboratively with a multidisciplinary team to develop and implement patient care plans
- Conducting assessments, documenting progress, and modifying care plans as needed
- Assisting patients with exercises and hands-on therapy to manage pain and increase mobility
- Educating patients on their plan of care, established goals, and self-care practices
- Counseling family members on safe exercise techniques and supporting their loved ones
Requirements
To succeed in this role, you'll need:
- Graduation from an accredited Physical Therapist program and active PT license in Kansas
- CPR certification and knowledge of state and federal home health regulations
- Excellent clinical judgment, strong interpersonal skills, dexterity, and compassion
