As a Home Health Physical Therapist, your responsibilities will include:
- Providing home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
- Collaborating with a multidisciplinary team to develop and implement patient plans of care and goals
- Conducting assessments, documenting progress, and modifying care plans as needed
- Assisting patients with exercises and hands-on therapy to manage pain and increase mobility
- Educating patients on their plan of care, established goals, exercises, and self-care
- Counselling family members on the plan of care, therapy goals, and safely assisting with exercises
To be successful as a Home Health Physical Therapist, you must possess:
- A graduate degree from an accredited Physical Therapist Program and active PT license in Kansas
- CPR certification
- Knowledge of state and federal home health regulations
- Excellent clinical judgment, strong interpersonal skills, dexterity, and compassion
